Embury District Forms

Each local church is required to fill out a number of forms for the annual charge conference. These are in a standardized format developed by the General Church.

The following forms are those required to be returned to the Embury District Office, preferably electronically, or by mailing to: PO Box 189, Clifton Park, NY. Deadline for submitting forms is January 31, 2010.

You can input Local Church Leadership information to the Conference web site by using your church's dashboard.  Please call the District Office if you would like instrcutions on how to do this.
